Wegovy dosage: what the label schedule says
Quick answer. The Wegovy injection label (Novo Nordisk, semaglutide) gives a starting dose of 0.25 mg once a week for 4 weeks, then 0.5 mg, 1 mg and 1.7 mg, each for 4 weeks (weeks 1 to 16). From week 17 the adult weight-reduction maintenance dose is 1.7 mg or 2.4 mg, with 2.4 mg marked as recommended. If 2.4 mg has been tolerated for at least 4 weeks and more weight reduction is clinically indicated, the label allows up to 7.2 mg once a week in adults. The Wegovy tablet is separate: 1.5 mg daily for days 1 to 30, 4 mg for days 31 to 60, 9 mg for days 61 to 90, then 25 mg daily.
The Wegovy injection schedule in the label
One Wegovy label, revised June 2026, covers both the injection and the tablets. These rows are from its injection dosing section. Wegovy is the weight-management brand of semaglutide, and the same table applies to adults and to children and teens 12 and older.
| Weeks | Once-weekly dose under the skin | What the label calls it |
|---|---|---|
| 1 to 4 | 0.25 mg | Starting dose |
| 5 to 8 | 0.5 mg | Escalation step |
| 9 to 12 | 1 mg | Escalation step |
| 13 to 16 | 1.7 mg | Escalation step, and a listed maintenance dose |
| 17 onward | The maintenance dose for the use being treated (see the next table) | Maintenance |
If a dose is not tolerated during escalation, the label says to consider delaying escalation for 4 weeks. The label says to give the injection once a week on the same day each week, at any time of day, with or without meals. The time of day and the injection site can change without a dose adjustment.
Maintenance and highest doses by use
| Use listed on the label | Maintenance dose the label lists | Highest dose |
|---|---|---|
| Weight reduction in adults | 1.7 mg or 2.4 mg once a week. The label marks 2.4 mg as recommended. | 7.2 mg once a week, only for adults who tolerate 2.4 mg for at least 4 weeks when more weight reduction is clinically indicated. The 7.2 mg dose comes in a pen the label calls Wegovy HD. |
| Weight reduction in children and teens 12 and older | 2.4 mg (recommended) or 1.7 mg once a week. | 2.4 mg |
| Lowering the risk of major cardiovascular events in adults with heart disease and obesity or overweight | 2.4 mg (recommended) or 1.7 mg once a week. | 2.4 mg |
| Noncirrhotic MASH (a liver disease) with moderate to advanced scarring, in adults | 2.4 mg once a week. If that is not tolerated, the label says the dose can be lowered to 1.7 mg and re-escalation to 2.4 mg considered. | 2.4 mg |
The MASH use was approved under the FDA's accelerated approval pathway, which means continued approval may depend on a confirmatory trial. The label describes the 7.2 mg dose only for adult weight reduction. The Wegovy tablet label text covers adults only.
Wegovy pens, syringes and the FlexTouch pen
| Form | Strengths listed | Notes from the label |
|---|---|---|
| Single-dose pen | 0.25 mg in 0.5 mL, 0.5 mg in 0.5 mL, 1 mg in 0.5 mL, 1.7 mg in 0.75 mL, 2.4 mg in 0.75 mL, and 7.2 mg in 0.75 mL (listed as Wegovy HD) | One dose per pen. |
| Single-dose prefilled syringe | 0.25 mg in 0.5 mL, 0.5 mg in 0.5 mL, 1 mg in 0.5 mL, 1.7 mg in 0.75 mL and 2.4 mg in 0.75 mL | No 7.2 mg syringe is listed. |
| Wegovy FlexTouch (single-patient-use pen) | 9.6 mg in 3 mL (3.2 mg per mL), delivering four 2.4 mg doses | Listed at the 2.4 mg strength only. The label says it is not recommended for self-injection by people who are visually impaired, and it must never be shared, even if the needle is changed. |
The label says patients and caregivers should be trained on the exact form they receive, and should read the instructions for use again if the form changes. See the brand-name pens page for what the maker's written instructions say about the pen.
The Wegovy tablet schedule
The Wegovy tablet is a different product from the injection and from the Ozempic and Rybelsus tablets. It contains semaglutide and is taken once a day. The label lists it for adults only.
| Days | Once-daily dose | What the label calls it |
|---|---|---|
| 1 to 30 | 1.5 mg | Starting dose |
| 31 to 60 | 4 mg | Escalation step |
| 61 to 90 | 9 mg | Escalation step |
| 91 onward | 25 mg | Maintenance dose |
The label says the tablet is swallowed in the morning on an empty stomach with up to 4 ounces of plain water, and that the person waits at least 30 minutes before eating, drinking or taking other oral medicines. For how the tablet compares with the injection in trials, see GLP-1 pill or shot. The other daily weight-management tablet, Foundayo, is a different medicine with its own steps (a 30-day minimum per step, with or without food), on the Foundayo dosage page.
What the label says about a missed dose
| Product | Situation | What the label says |
|---|---|---|
| Injection | One dose missed and the next scheduled dose is more than 2 days away | Give the missed injection as soon as possible. |
| Injection | One dose missed and the next scheduled dose is less than 2 days away | Do not give the missed dose. Resume on the regularly scheduled day of the week. |
| Injection | Two or more consecutive doses missed | The label says to reinitiate dose escalation at a lower dosage, to reduce the risk of digestive side effects. |
| Tablet | A dose is missed | Skip the missed dose and take the next dose the following day. |
The two-or-more-doses wording is from the label revised June 2026. An earlier copy of the label on the FDA site (May 2026) worded this differently, saying to resume dosing as scheduled or, if needed, to reinitiate and follow the escalation schedule. That is why this page names the version. The label does not state a minimum gap if the weekly injection day is changed.
Switching between the Wegovy injection and tablets
This is the one switching instruction in the Wegovy label. From the 2.4 mg injection to the 25 mg tablet, the label says to start the tablet one week after stopping the injection. From the 25 mg tablet to the injection, it says to start the 2.4 mg injection the day after the last tablet. If the 25 mg tablet is not tolerated, the label says to consider the 1.7 mg injection.
The label says that using Wegovy with other semaglutide products or any other GLP-1 receptor agonist is not recommended. It gives no text on moving to or from Zepbound, Mounjaro or another medicine. See the switching page, and Ozempic vs Wegovy for how the two semaglutide brands differ.
What the Wegovy dose trials tested
Trial results are group averages from the study named, not promises for any one person. The last column names the analysis approach, because changes the numbers.
- In STEP UP, side effects also rose with the dose. Digestive events were reported by 70.8% on 7.2 mg, 61.2% on 2.4 mg and 42.8% on placebo. Altered skin sensation (dysaesthesia) was reported by 22.9%, 6.0% and 0.5%. Serious adverse events were 6.8%, 10.9% and 5.5%.
- STEP UP compared assigned doses. The label says 7.2 mg is for adults who have already tolerated 2.4 mg for at least 4 weeks and need more weight reduction, which is a different starting point from the trial arms.
- The tablet trial tested the 25 mg dose only. It shows nothing about the 1.5 mg, 4 mg or 9 mg steps beyond their place in the label schedule.
| Trial | People and length | Dose tested | Result | Analysis approach |
|---|---|---|---|---|
| STEP 1 | 1,961 adults with obesity, or overweight plus a weight-related condition, without diabetes. 68 weeks. | 2.4 mg once a week, against placebo | Average weight change: -14.9% on semaglutide and -2.4% on placebo. | Regardless of treatment discontinuation or rescue treatment. |
| STEP 5 | 304 adults with obesity or overweight, without diabetes. 104 weeks. | 2.4 mg once a week, against placebo | Average weight change: -15.2% on semaglutide and -2.6% on placebo. | Regardless of treatment discontinuation or rescue treatment. |
| STEP UP | 1,407 adults with a body mass index of 30 or higher, without diabetes. 72 weeks. | 7.2 mg and 2.4 mg once a week, against placebo | Average weight change: -18.7% on 7.2 mg, -15.6% on 2.4 mg and -3.9% on placebo. The 7.2 mg group lost 3.1 percentage points more than the 2.4 mg group (95% confidence interval 1.6 to 4.7). | Treatment-policy estimand: regardless of whether people stayed on treatment. |
| STEP 2 | 1,210 adults with overweight or obesity and type 2 diabetes. 68 weeks. | 2.4 mg and 1.0 mg once a week, against placebo | Average weight change on 2.4 mg: -9.6%, against -3.4% on placebo. The abstract gives no weight result for the 1.0 mg group. | Intention to treat. The abstract does not name an estimand. |
| STEP TEENS | 201 adolescents aged 12 to under 18 with obesity, or overweight plus a weight-related condition. 68 weeks. | 2.4 mg once a week, against placebo | Average change in body mass index: -16.1% on semaglutide and +0.6% on placebo. | The abstract does not name the approach. |
| OASIS 4 | 307 adults with obesity, or overweight plus a complication, without diabetes. 64 weeks. | 25 mg tablet once a day, against placebo | Estimated average weight change: -13.6% on the tablet and -2.2% on placebo. | Not stated in the abstract. |

What the makers list for the cash price
Novo Nordisk's NovoCare page listed these self-pay prices when the Atlas read it in October 2026. The pen prices are per box of four pens. The tablet prices are per bottle of 30 tablets.
| Product and dose | Price listed for a month |
|---|---|
| Wegovy pen, first two fills at 0.25 mg and 0.5 mg, new patients only | $199, through December 31, 2026 |
| Wegovy pen, doses from 0.25 mg to 2.4 mg | $349 |
| Wegovy pen, 7.2 mg (Wegovy HD) | $399 |
| Wegovy tablet, 1.5 mg | $149 |
| Wegovy tablet, 4 mg | $199 |
| Wegovy tablet, 9 mg and 25 mg | $299 |
The page says eligibility rules apply, the company can change or cancel the program at any time, and self-pay amounts do not count toward a deductible or out-of-pocket limit. The Atlas has not independently verified the prices. See paying for GLP-1 medicines and the cost tool.
Compounded semaglutide is a different matter
The strengths, steps and missed-dose rules on this page belong to the approved Wegovy product. Compounded versions are made by pharmacies, are not FDA-approved and are not reviewed by the FDA for safety, effectiveness or quality before they are sold. In July 2024 the FDA issued an alert about dosing errors with compounded injectable semaglutide products. See compounded versus brand-name medicines and GLP-1 microdosing.
Quick answers
- What is the Wegovy dosage?
- For the injection, the label starts at 0.25 mg once a week for 4 weeks, then 0.5 mg, 1 mg and 1.7 mg for 4 weeks each. From week 17 the adult weight-reduction maintenance dose is 1.7 mg or 2.4 mg, with 2.4 mg recommended. A prescriber sets the plan.
- What is the maximum Wegovy dose?
- For adult weight reduction, the label allows up to 7.2 mg once a week for adults who have tolerated 2.4 mg for at least 4 weeks and need more weight reduction. For the other listed uses, 2.4 mg is the highest dose the label describes.
- What is the Wegovy 2.4 mg dose?
- It is the maintenance dose the label marks as recommended for adult weight reduction, for lowering cardiovascular risk and for MASH. In STEP 1, adults on 2.4 mg once a week lost an average of 14.9% of body weight over 68 weeks, against 2.4% on placebo.
- How long is each Wegovy step?
- Four weeks each: 0.25 mg for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12 and 1.7 mg for weeks 13 to 16. If a dose is not tolerated, the label says to consider delaying escalation for 4 weeks.
- What is the Wegovy pill dosage?
- The tablet label starts at 1.5 mg once a day for 30 days, then 4 mg for 30 days, then 9 mg for 30 days, then 25 mg once a day as the maintenance dose. It is for adults.
- What if a Wegovy injection is missed?
- If the next scheduled dose is more than 2 days away, the label says to give the missed injection as soon as possible. If it is less than 2 days away, do not give it and resume on the regular day. If two or more consecutive doses are missed, the June 2026 label says to reinitiate dose escalation at a lower dosage.
- What are the Wegovy pen strengths?
- The single-dose pens are 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg and 7.2 mg (Wegovy HD). There is also a multi-dose FlexTouch pen holding four 2.4 mg doses. The label also lists single-dose syringes from 0.25 mg to 2.4 mg.
- Is 7.2 mg Wegovy different from Wegovy HD?
- The label lists the 7.2 mg pen as Wegovy HD. See the Wegovy HD page for what the Atlas has on that product.
- Is the Wegovy dose the same as the Ozempic dose?
- Both contain semaglutide, and both labels list a weekly injection dose of 1 mg among their steps, but the products have separate labels, different schedules and different approved uses. The Atlas found no label text that makes one product's dose a replacement for the other's.
Questions to ask
- What maintenance dose does the label allow for the reason I am being treated, and how will we decide?
- If I miss a dose, who do I call, and what does your office want me to do?
- Which form will I get (single-dose pen, syringe, FlexTouch or tablet), and who will teach me to use it?
- What will each dose step cost me, with my coverage?
- What is the plan if I cannot tolerate a step?
What is still unknown
- The Atlas read the injection and tablet sections of the label revised June 2026 and compared them with the May 2026 copy on the FDA site. The wording on two or more missed doses differs between the two, so the current label is the one to check.
- The label does not state a minimum gap if the weekly injection day is changed. The Atlas read only the dosing sections.
- The Atlas read the published abstracts of the trials, not the full papers. STEP 2, STEP TEENS and OASIS 4 do not name their analysis approach in the abstract.
- STEP UP tested assigned doses and the Atlas record gives no detail on how people reached 7.2 mg. Whether and when a person needs 7.2 mg is a prescriber decision.
- The Atlas has not summarized the trials behind the heart and MASH uses on the label.
- Self-pay prices are as listed in October 2026 and change often.
